Bichectomy

The procedure is usually performed under local anesthesia or sedation, depending on the extent of the surgery. During the procedure, the surgeon makes small incisions inside the mouth and uses a small instrument to remove the buccal fat pads. The incisions are closed with dissolvable sutures and the procedure usually lasts about an hour.

Patients may experience some discomfort and swelling after the procedure, but the pain is usually controlled with over-the-counter or prescription pain relievers.

Most patients are able to return to work and other normal activities one week after surgery, but it may take several weeks for the swelling to completely disappear.

There may be a slight increase in subcutaneous facial fat but it will not give the appearance of a bichat pocket.

With or without bichats, the skin becomes saggy over the years due to decreased collagen production.
